Pre-therapeutical standard GFR of 40–49 ml/min/1.73m2, previous PRRT or multiple risk factors (e.g. nephrotoxic chemotherapy, longstanding diabetes or hypertension)

3 l solution over 12 h starting ½ hour pre-treatment

2 l solution and 1 l saline infused over 12 h starting 1 h pre-treatment. Additionally ½ l solution infused over 2 h starting 24 h post-treatment

Pre-therapeutical standard GFR of 30–39 ml/min/1.73m2

4 l solution over about 24 h starting ½ hour pre-treatment (150–170 ml/h)

3 l solution and 1–2 l saline infused over 24–36 h starting 1 h pre-treatment. Additionally ½ l solution infused over 2 h starting 48 h post-treatment
